Liability Waiver

At Made 4 Kids Hockey Training Inc., the safety and well-being of your children are our top priorities. To ensure a secure and enjoyable experience for everyone involved, we require all parents and guardians to complete a Waiver of Liability and Release form before their children can participate in our training programs.

This waiver is a standard procedure that helps us manage risk and maintain a safe environment for all participants. By completing this form, you acknowledge and agree to the terms and conditions outlined, allowing your child to engage fully in our hockey training sessions. Please note that due to manual verification, it may take up to 24 hours for your waiver to be approved. 

Please take a few moments to carefully read and fill out the Waiver of Liability and Release form. Your cooperation is essential for us to proceed with your child’s training and provide them with the best possible experience.
